The Partnership
The GMHF Program is co-ordinated by a Central Team at Great Places Housing Association and they are supported by Zone Leads, Team Leaders and Housing First Workers who work for the Delivery Partners in specific Local Authorities across Greater Manchester.
The partnership is based on the following principles
Cross sectoral
Partnership incorporating housing, health services, the community and voluntary sector and people with lived experience.
Equitable
Open and fair service treating people as equal citizens with an open and fair service provided by all the delivery partners. Caps on delivery and equal salaries for all frontline workers.
Local capacity building
Good practice sharing, training and continuous improvement culture.
Co-produced
People with lived experience at the heart of model design and will continue to lead its development
High Fidelity 
Aligned to the seven Housing First principles ensuring improved outcomes for people.

How the partnership works
Each Delivery Partner has a Housing First team consisting of a Zone Lead with overall responsibility for the services delivered in the zone. Team Lead(s) and Housing First Workers (HFWs), who deliver the bespoke support for people on the program aligned to the Housing First Principle (link). The teams also have a Dual Diagnosis practitioner who provides support to staff and people on the program around mental and physical health and substance misuse problems.
 
Each Team Lead manages between six  to seven Housing First Workers, Each HFW has their caseloads limited to six to seven people to allow for high levels of individually focussed (Person Centred) bespoke and intensive support to be delivered to support people goals and aspirations as they move away from homelessness.
