Referring people to the pilot
To be eligible for the Housing First Programme in Greater Manchester people have to fit the definition of being a person facing multiple need and exclusion and be someone who could not be accommodated through an existing traditional pathway.
GMHF is for people aged 18+ who are homeless or at risk of homelessness and be willing to try taking on a tenancy and be in receipt of benefits, willing to apply for benefits or have an income to cover the rent.
The referrer should be someone who knows the person and can complete the Assessment Form (below) as fully as possible; they will also be required to provide an initial introduction between the Housing First Worker and the person if they are accepted.
Prior to completing the Assessment Form the GMHF programme should be explained to the
person, the FAQ’s sheet can help with this. Permission to refer must be gained from the person or the referral will be rejected.
The Privacy Notice should be given to and explained to the person.
When complete the Assessment Form should be emailed to the contact email for the Local Authority (LA) they are applying to. Please make it clear in the email that the referral is for the GM Housing First programme.
Referrals are considered at a multi agency panel which in most LA’s is held monthly.
Each LA has a limited monthly referral allocation and they may need to prioritise the referrals they receive.
If accepted the LA will contact the GMHF Central Team who will pass the referral to the local Delivery Partner (DP); the DP will contact the referrer for an introduction.
If the referral is not accepted for the Housing First programme the LA will provide this feedback to the referrer detailing the reasons and providing advice as to other options.
